If you have locked your SIM by incorrectly entering your SIM PIN three times, your phone will ask you to input a PUK code, which you can obtain through your My.plan account log in.
To view your PUK code(s), go to:
CONTROLS > DEVICE > PUK CODE > DEVICE CONTROLS: PUK CODES
Use PUK1. If you incorrectly insert PUK1 three times, you will need to use PUK2
Once you are in possession of your PUK code, enter it where indicated into your phone and you will be prompted to set a new PIN code.
If you incorrectly enter the PUK2 code 10 times, your SIM card is locked forever and you'll need a replacement.
